What are we doing and why?
NHS South Tees Clinical Commissioning Group (CCG) embarked on a public consultation about services for the vulnerable and elderly and those living with long-term health conditions such as diabetes, heart disease or chronic obstructive pulmonary disease (COPD).

The Governing Body will agree a decision on the future of the IMProVE programme at an Extraordinary meeting on the 15th October 2014
This work is part of the IMProVE project. IMProVE stands for integrated management and proactive care for the vulnerable and elderly
We believe that by investing in more community-based services, closer to where people live, we will be able to improve the quality of care and respond more effectively to existing and future health needs.
For around two years we have been working with GPs, hospital doctors, managers and local authority partners to consider the challenges we face in meeting the needs of a growing population of older patients with long-term conditions and other health and social care requirements. We have also engaged the public, service users and carers to find out what they think of existing services and our vision for future development.
